Appointment Fees
Cash, checks, debit or credit card are accepted forms of payment. Payment is rendered at the appointment time. We are currently accepting health insurance billing from Harrington Health Insurance, Aetna, Anthem BlueCross BlueShield, CIGNA and Health Net. Patient is responsible for all fees not covered or paid for by the insurance company within 30 days notice is received from the insurance company. Prices may change without prior notification. In the event that a fee is left unpaid, and you do not pay your billed amount within 30 days, your bill will be sent to a collection agency or appropriate legal action may be taken. Lotus Medical & Health Clinic assumes no responsibility for actions which may occur as a result of an outstanding bill being sent to a collection agency or legal action necessary to resolve your medical bill.
Appointment Policy
We would like to provide you with excellent service and treatment(s). In order to do so, we need you to be present for your appointment time. All appointments are 50 min. in duration. All appointments are scheduled in the Pacific Standard Time zone. The following is the clinic’s policy regarding appointments:
Late Arrival
Late arrival or late call in reduces appointment time, not session cost. Late time cannot be made up for by adding time on to the end of treatment time or at another date. Continual or frequent late arrival may result in termination of treatments.
Rescheduling
Rescheduling of an appointment time may occur if the client reschedules with no less than 24 hours notice. All appointments within the 24 hours prior to their session time are considered a fix appointment and the patient is fully committed to scheduled appointment and associated fees.
Cancellation and Missed Treatment
Cancellation of an appointment must occur no less than 48 hours prior to the appointment time. If an appointment is cancelled more than 48 hours prior to the appointment time, there will be no charge for the full cost of the treatment. If an appointment is canceled less than 48 hours prior to the appointment time, the full appointment fee will be billed to the client. If an appointment is missed, the client will be billed the full appointment amount. Continual or frequent cancelation or missed appointment may result in the termination of treatment.
Herbal Medicine
Herbal medicines and products are prescribed and sold for immediate consumption or use as indicated by the prescribing practitioner. Herbal products may not be returned or refunded if opened or if unopened products are more than 30 days after purchase. If within the initial 30 day refund period, the product(s) must be unopened and receipt must be present at time of return and/or refund. If a product is defective, and it is within 30 days after purchase, a refund will be administered. Individualized herbal formulas are prescribed within 10 business days of treatment and/or consultation. All individualized herbal formulas are subject to the terms, agreements and policies of Crane Herb Company.
Scents and Perfumes
Due to the sensitivities of other patients, please do not wear any scents, scented items, materials or bring scented products to appointments.
HIPAA Privacy Practices and Procedures
Lotus Medical & Health Clinic (“LMHC”) may only use or disclose your Protected Health Information (“PHI”), as defined in the Health Insurance Portability and Accountability Act of 1996 (HIPAA), for the purpose of making or obtaining payment for your care, conducting its health care operations, or as otherwise allowed by HIPAA. Protected Health Information is considered to be individually identifiable health information that is transmitted or maintained by LMHC in any form. LMHC will only use or disclose PHI in a manner that complies with HIPAA. All responses to requests for PHI will be limited to the minimum amount of information needed to accomplish the purpose of the request or disclosure. An individual may authorize use, request restrictions, inspect his or her records, and amend and request an accounting of disclosures of his/her PHI. The Notice of HIPAA Privacy Practices describes in more detail how an individual’s PHI may be used and disclosed. A copy of Clinic Policies, Notice of HIPAA Privacy Practices and Notice of HIPAA Privacy Procedures are available upon request.
